Steven J. Fowler, MD
Has been a sub specialist in cardiac rhythm disorders (electrophysiology) for 15 years; in practice for over 20 years. He joins Golden State Heart & Vascular from Montage Medical Group after coming from New York City 5 years ago.
He was at New York University Langone Medical Center, where he served as Assistant Professor of Medicine in the Division of Cardiology & Director of the Inherited Arrhythmia Clinic and was part of the Cardiovascular Genetics Program after specialized training in Italy, since 2008. He specializes in treatment of irregular heartbeat, or arrhythmia, for which he has performed over 10,000 procedures within the past 15 years.
Dr. Fowler completed his residency in internal medicine at Scripps in San Diego. He did a fellowship in cardiovascular disease at UC Irvine, and was chief fellow in Clinical Cardiac Electrophysiology at NYU. Dr. Fowler is board-certified in Clinic Cardiac Electrophysiology and Cardiovascular Disease and serves as the Medical Director of Community Hospital’s Electrophysiology Laboratory.
Colm J Murphy, MD, FRCPC
Dr. Colm Murphy is a passionate and skilled Interventional Cardiologist with expertise in cardiac and vascular disease.
A subspecialist in interventional cardiology and echocardiography since 2013.
In practice for the last 10 years, most recently at Sutter Memorial Hospital and Doctors Medical Center, Modesto, CA, before joining Golden State Heart and Vascular.
A former CEO of a medical technology startup, Openxmed, based in Alberta, Canada, developing decision making algorithms for physicians.

Nima Patel, PA
Nima Patel is a highly skilled Physician Assistant dedicated to delivering exceptional care across all aspects of patient management. Her expertise spans new patient examinations, minor procedures, vascular ultrasounds, assisting in the operating room, triaging patient needs, and managing postoperative follow-ups. Nima is well-versed in vascular disease and holds formal certifications in vascular ultrasonography (RVS) and wound care (CWS).
With a steadfast commitment to providing patient care with integrity, compassion, and professionalism, Nima plays an integral role in ensuring comprehensive and personalized treatment for each patient.
Born and raised in San Francisco, Nima pursued her passion for medicine early on. She earned her Bachelor of Science from St. Mary’s College in Moraga, California, and completed her Master’s degree in the Physician Assistant program at Touro University in Nevada. During her academic journey, she gained valuable hands-on experience working as an Emergency Medical Technician (EMT).
Nima’s extensive knowledge, coupled with her dedication to advancing patient outcomes, makes her an indispensable part of our team.
Lilian Pham, PA
Lilian Pham is a recent Physician Assistant graduate from Midwestern University in Glendale, AZ. She is eager and excited to start her career with GSHAV and hopes to sharpen her skills under the guidance of our talented physicians. She is devoted to providing compassionate and professional care to her community. Lily was born and raised in San Jose and has ingratiated her younger years around volunteering in hospitals here and in Los Angeles, where she completed her BS in Psychobiology at UCLA. In her free time, she loves traveling, trying out new restaurants, swimming, and weight lifting at the gym.
Allon Rafael , MD

Dr. Rafael is a cardiologist and electrophysiologist (cardiac arrhythmia specialist), treating patients with heart rhythm disorders. He received his undergraduate degree from U.C.L.A. and his medical degree from Tel Aviv University in Israel. Dr. Rafael is board certified in Internal Medicine, Cardiology, and Cardiac Electrophysiology (EP). He trained in EP at the Massachusetts General Hospital in Boston – the original and largest teaching hospital of Harvard Medical School – then returned to the Bay Area in 2016 to practice medicine.
Dr. Rafael’s focus is on the treatment of abnormal heart rhythms including dangerously slow and fast arrhythmias. He is trained in advanced catheter ablation techniques, as well as implantation of cardiac devices such as pacemakers, defibrillators, and cardiac resynchronization devices for treatment of congestive heart failure. He is one of only a few doctors in the community who implant the Watchman device (a solution for patients who cannot safely take blood thinners), and was the first doctor to implant the leadless pacemaker in San Jose.
In his spare time, Dr. Rafael enjoys rooting for the Giants, biking, golf, SCUBA diving, and spending time with his wife and daughters.
Melinda Stevens
Melinda Stevens is our Chief Operating Officer. She brings executive-level experience in consulting, coaching, and training physicians, managers, and medical staff nationwide. Her expertise spans multiple specialties, including vascular surgery, neurology, neurosurgery, orthopedics, otolaryngology, cardiology, and dermatology. Melinda is highly skilled in all aspects of practice management, from marketing and financial oversight to operational efficiency, leadership, governance, human resources, and patient communication.
Beginning her career in the clinical realm, specializing in respiratory care, Melinda pursued further education, earning both her clinical qualifications and an MBA from Vanderbilt University. She dedicated over 20 years as a CEO on the East Coast, where she built a proven track record in executive-level finance, marketing, operations, and strategic leadership.
